I, __________________________ on behalf of the _________________________Surname Project and its participants, authorize Family Tree DNA to collect use the genealogies I provide, and analyze those various family branches to calculate polymorphism (aka mutations) in my various group's lineages to determine the mutation rate.

I understand that the individual’s genealogies will be kept completely private and will be destroyed after the completion of the project. The sole purpose for collecting genealogical data is to authenticate the number of generations between our project members as we compare actual mutations to the number of actual generations.

I understand that while individual surname projects will not receive specific benefit from the study, all projects will be the common beneficiary of the aggregate work which will validate the mutational rate within families, and will investigate for potentially different mutation rates within families from various different male populations.
